We are Family Serving Families!
In 2013, Covenant Care expanded its skilled nursing services with an entry into the Home Health sector by purchasing Elevate Home Health in Long Beach. Subsequently, in February of 2015, Covenant Care acquired Focus Home Health. Our team is passionate about helping people live healthier, happier lives.

Elevate Home Health operates two levels of in-home care.
Home Healthcare
We provide a unique focus on rehabilitative and chronic disease management programs to our patients in their home or community setting. We offer a wide range of healthcare services, all of which are uniquely tailored to our patients needs. Whether a patient is recovering from a surgery or dealing with a newly diagnosed or chronic illness, Elevate Home Health is here for you. Our Home Health services include skilled nursing, physical therapy, occupational therapy, speech therapy, medical social work and in-home aide services. The services provided to you will be determined by your doctor.

Our Personal Care services provide in-home care that can help you or a loved one live a more enriched life. Our caregivers will ensure that you or your loved one is safe and cared for while providing assistance with a variety of personal care activities.
